ZiNGO Surgical Guide Service Terms

Effective date: 10/10/2026

1. Starting and approving your case

Before planning or design begins, we require a signed Main RX, any required Full-Arch Addendum, acceptance of these terms, and suitable records needed to start. Details marked for determination during planning may be confirmed later.

Before any planning or design work begins, we agree with you in writing on the applicable rates, initial case scope, and a cancellation fee limit. This limit applies if the case is cancelled after work starts but before the first treatment plan is sent for review.

The final guide configuration, included components, and total service price are agreed before fabrication. Additional items, such as a separate bone reduction guide or provisional restoration, are included only if stated in the agreed quote. Requested delivery dates are subject to ZiNGO confirmation.

The RX and addendum authorize planning and design only. Fabrication requires written approval, including by email, of the final implant plan and guide design from the dentist identified on the RX as approving that arch’s plan. Changes after approval require renewed written approval. Clinical decisions remain the treating clinician’s responsibility.

2. Planning fees — per arch

These fees cover planning and design. Fees, approval, cancellation, credits, and the 30-day approval period apply separately to each arch.

Standard guide — 1–2 implants: $140

Each additional implant in a standard guide: +$28

Full-arch, one-piece (nonstackable) guide: $250

Full-arch, stackable / multi-stage guide: $360

Full-arch fees are flat per arch, regardless of implant count. Fixation pins are not counted as implants.

After a plan has been sent, the standard planning fee is based on the implant count in the most recent plan sent for approval when cancellation occurs or the approval period expires without approval, whichever first triggers the fee.

If the count is initially unknown, we state the calculated fee with the first plan. A revised plan changing the count includes the updated fee. For example, three implants in a standard guide carry a $168 planning fee.

Changes to the guide type or scope and related charges are agreed before work on the changed scope begins.

3. When charges apply

  • Before planning or design work begins: cancellation is free.

  • After work begins, before the first plan is sent: completed planning and design work is billed, up to the written limit agreed before work began.

  • After the first plan is sent, before fabrication begins: cancellation results in the full applicable planning fee, even if the plan has already been approved.

  • If the 30-day approval period expires without approval: the full applicable planning fee is invoiced, even without a cancellation request.

  • After approval and the start of fabrication: cancellation is billed at the agreed guide service price, plus previously approved, nonreturnable component purchases not already included in that price. No additional planning or cancellation fee is added.

Cancellation and expiration of the approval period are alternative grounds for the same planning fee. They do not create two charges.

4. Included revisions

Each arch includes the initial treatment plan and guide design, plus four rounds of revisions at no additional charge, within the agreed case scope and before final approval.

One round consists of one consolidated set of requested changes followed by our delivery of the revised plan and/or guide design. There is no limit on the number of individual comments within a round.

Corrections of ZiNGO errors or failure to follow agreed instructions are free and do not count toward the four rounds.

Additional rounds and any related charges are agreed in writing before further revision work begins. New scans or scope changes requiring a new design are handled under Section 6.

5. How the 30-day period works

The approval period is 30 calendar days, starting when the first treatment plan for that arch is sent for review. Revision requests do not restart it.

Time spent correcting ZiNGO errors and delays caused by ZiNGO do not count toward the 30-day period.

Expiration of the period does not constitute approval or authorize fabrication.

6. Credits and subsequent changes

If the same case proceeds without a new design, the planning fee is included in the guide service price. Previously invoiced amounts are accounted for in the final billing, and payments already made are credited. The same planning work is never billed twice.

Revisions within the agreed case scope do not, by themselves, remove this credit.

New scans or scope changes requiring a new design are quoted and agreed upon separately before additional work begins. Changes to an already invoiced scope and any related additional charges also require prior agreement.

7. Corrections and payment

Issues caused by ZiNGO errors or failure to follow agreed instructions are handled as corrections, rather than standard cancellations.

Invoices are payable within 30 days of the invoice date, unless otherwise agreed in writing. This is separate from the plan approval period.

These terms supplement the ZiNGO Lab Terms & Payment Policy. For surgical guide planning and cancellation, these service-specific terms take precedence.
